The Polysystems concept is becoming a reality across England. Driving innovation and efficiency through partnership will be key to making primary care deliver more for patients and create a local NHS network that will improve patient choice.

Delivering change

Developing polysystems into robust and sustainable vehicles for change will require the engagement and commitment of stakeholders, from patients through to clinical staff. Key to this is:

  • process mapping
  • stakeholder engagement
  • clinical and referral pathway redesign
  • an estates utilisation strategy
  • development of well designed services that can provide more complex care in the community

At Mace we will work with PCT organisations to develop local services. This may include additional community based urgent care, diagnostics and outpatient services, which will improve access to a wide variety of services for patients in locations more convenient for them.

Polysystems can deliver improvements in patient care by creating partnerships between existing primary care services, community pharmacies, social services and secondary care teams which drive service change, and create a seamless health service delivered in the community.
